Recently, the National Health and Family Planning Commission, the Ministry of Public Security, the Ministry of Transport, the China Civil Aviation Administration, the China Railway Corporation, and the China Red Cross Society jointly issued the "Notice on Establishing a Green Passage for Human Donor Organs Transshipment", requesting the establishment of human donations. Organs transport green channels, clarify the responsibilities of all parties, standardize and smooth the transshipment process, form institutional arrangements, improve transshipment efficiency, ensure transshipment safety, reduce organ waste caused by transportation, and protect the life and safety of human donors. The relevant points are now interpreted as follows: I. Drafting background Organ transplantation is a great achievement in the development of human medicine, saving the lives of countless end-stage diseases, and the prerequisite for the successful implementation of organ transplantation is that there are organs for transplantation. At present, China has initially established a human organ donation system that is in line with the national conditions and participated by the Chinese Red Cross as a third party, and has achieved initial results. As of May 5, 2016, 7101 cases of organ donation and 19,417 large organs were donated after the death of citizens. Among them, 2,766 donations were completed in 2015, and 7,785 large organs were donated, exceeding the sum of the contributions in 2013 and 2014. China's annual donation has ranked first in Asia and third in the world. In September 2013, China officially launched the Chinese Human Organ Distribution and Sharing Computer System (hereinafter referred to as COTRS) to allocate donated organs. COTRS strictly follows the organ allocation policy, uses technical means to eliminate and monitor human intervention to the maximum extent, and ranks patients with internationally recognized objective medical indicators such as patient urgency and donor-acceptance matching, to achieve automated organ matching and sharing. Ensure the scientific, fair and equitable distribution of organs. National networking has greatly improved organ utilization, and 13.1% of organs achieved cross-provincial patient matching and sharing within an acceptable cold ischemic time frame. The relatively frequent sharing of organs for children has reached a national share of 22.6%, making more effective treatment for children with organ failure throughout the country. With the further advancement of organ donation in China, the number and proportion of donated organs nationwide matching will increase. Although the sharing of organs is conducive to the realization of fair and efficient organ allocation, subject to current medical technology, transplantable organs have a maximum ischemic time tolerated, for example, the upper limit of liver tolerance to cold ischemia is 12 hours. The incidence of complications such as primary liver transplantation, delayed recovery of liver function, and biliary ischemic injury was significantly increased in the time recipients, and the survival rate of the recipients was significantly reduced. The upper limit of renal tolerance to cold ischemia is about 24 hours, the upper limit of cardiac tolerance to cold ischemia is about 6-8 hours, and the upper limit of lung tolerance to cold ischemia is about 8-12 hours. Even in a tolerable time, the longer the ischemic time, the worse the quality of the organ and the prognosis of the organ recipient. So organ transport is a race of life and time. Limited by China's vast land area and social and economic development level, and in order to reduce the total cost of organ transplants in patients, most of China's use of medical personnel to transport donated organs through civilian flights, high-speed rail and road transport. There are many uncertain factors in the transshipment process, and the trans-shipment time is long, which has an adverse effect on the organ quality. Organ waste caused by the transport problem also occurs from time to time. To this end, the relevant departments have coordinated many times to provide guarantees for human organ donation, and some NPC deputies and CPPCC members have also proposed and proposed the establishment of human organ transport. Establishing a green channel for human donor organ transport, shortening organ transport time, improving the quality of transplanted organs, and reducing organ waste have become the eager expectations of medical workers, end-stage organ failure patients and the general public. Second, the drafting process Since the death of organ donation in the aftermath of the death of our citizens, the National Health and Family Planning Commission, the Ministry of Public Security, the Ministry of Transport, the Civil Aviation Administration of China, the China Railway Corporation and the China Red Cross Society have carried out various forms of business cooperation to jointly guarantee The human body donates organ transport work, which forms a certain working mechanism and foundation. For example, in 2007, Shenzhen Airlines and relevant units signed the “Agreement on Deep Shipping to International Standardized Human Donation of Corneal Membrane and Organsâ€, and Southern Airlines and other civil aviation transport companies have also provided support and assistance for organ transport. With the continuous deepening of human organ donation work in China, the trans-regional and long-distance organ transport cases have gradually increased and become the norm in the normal work, the original working mechanism can not meet the actual needs. In 2015, the National Health and Family Planning Commission and the Ministry of Public Security, the Ministry of Transport, the Civil Aviation Administration of China, the China Railway Corporation, the China Red Cross Society and other departments strengthened the joint meeting and held a special meeting on human body donation of organ transport, discussing the decision in the original On the basis of working mechanism, the human body will be allocated a green channel for organ donation, and institutional arrangements will be established to ensure the smooth transfer of donated organs and maintain the health of the people. Based on our experience in international organ transport management, based on China's national conditions and past work experience, we adhere to the principles of science, flexibility and practicality, and drafted the Notice on Establishing a Green Passage for Human Organ Donation Organs (Discussion Draft), and solicited opinions from relevant departments. On the basis of repeated revisions and improvements, the final draft form the "Notice on the establishment of a green channel for human donor organ transport."
