The sputum state refers to the speed-dependent stretch reflex. In stroke patients, the incidence rate is as high as 65%, which greatly affects the recovery of motor function. In the treatment of such patients with sputum control, stretching training is a simple and effective way, occupying an important position in the clinic, and some drafting devices have emerged. However, most of these drafting devices are concerned with the paralysis of the lower extremities, and only a small number of drafting devices focus on the paralysis of the wrist and hand. Moreover, these devices are generally cumbersome and complex, and are inconvenient for use by the patient.

Therefore, Sung et al. from the Department of Physical Medicine and Rehabilitation Medicine, Lingnan University, Korea, designed to control the effect of wrist-hand stretching devices in patients with chronic stroke and wrist and hand spasms. Related research and published the results in the online journal of Eur J Phys Rehabil Med February 2016.

This study was a prospective, single-blind, randomized, controlled clinical study. The subjects included in the study were patients treated in the outpatient department of the rehabilitation department.

A total of 21 subjects were enrolled and randomized into two groups, one in the intervention group (n = 11) and the other in the control group (n = 10). The drafting device consists of two parts, one of which is a circular plastic plate and the other part is a five-finger sleeve for fixing the fingers.

There are three types of stretching postures. In the posture 1, the middle finger tip is forward, the posture 1 is 90°, and the posture is 2, and the posture 2 is rotated 90° again, which is the posture 3. During the stretching process, each posture is held for 4 minutes, and the rest is 1 minute between the two postures. Therefore, one treatment time is 14 minutes.

The stretching training is 3 times a day, 6 days a week, and a course of treatment is 4 weeks. Three assessments are required throughout the treatment (first assessment before treatment, second assessment two weeks after treatment, and third assessment four weeks after treatment). The assessment includes the iliac state of the affected wrist and hand ( Modified Ashworth scale, MAS) and motor function (Fugl-Meyer motion assessment, FMA) and active joint mobility (AROM).

In the intervention group, there were significant differences between the three assessments, MAS and FMA in the wrist and hand. However, there was no significant difference in active joint mobility between the wrist and the hand. In the control group, there were no significant differences between the three evaluation results, MAS, FMA, and AROM.

The results of the study indicate that the application of this wrist-hand stretching device can effectively reduce paralysis and improve the functional prognosis of patients. In addition, the device is easy to use and carry, so it is more helpful to the patient.
